What documents or evidence are needed to raise a civic issue (e.g. grievance, petition)?

Quick answer: For most grievances you need: (1) Your name and contact (and address if required). (2) A clear description of the problem and where it is (ward, street, landmark). (3) A photo or document if it helps (e.g. photo of pothole, copy of earlier complaint). You usually do not need a formal application form — the grievance portal has fields to fill.
